We'll start with a truck symbol from Microsoft's Webdings symbol font shown to the right. The keyboard equivalent is h. The size is 185 points. Place the ambulance inside a rectangle as shown.

Select the symbol and Convert to Editable Shapes (Arrange menu).

Select the rectangle and the symbol, set the outline to none, and Export the two objects using the WMF (Windows Meta File) file option. Name the file truck.wmf

Note: The rectangle should have no fill or outline.
