Using the Fill Tool, apply a two-color Linear Fill as shown on the left.

New users, select the rectangle, then the Fill Tool, from the drop - down list (on the Infobar) select Linear. Drag colors from the screen palette to either end of the fill path. You can drag the fill path to alter the fill direction.

Add an eye symbol using Microsoft's Webdings font. The keyboard character is N. Make the size 180 points and apply a 20% black fill.
